can't run mplex

Hi.. I'm having trouble with mplex, I have tried to compile mjpegtools
1.4 on my Mandrake 8.0 box, but I keep running into missing files on
my system, (.h files largest hang-up = forms.h) anyway so I
downloaded the rpm for rh7 and it installs okay.. but when I try to
run mplex in my directory witht eh 2 streams I get this:

any ideas?
